Chronic osteomyelitis is an infection in a bone. It may be an incurable problem.
What happens with bone infections (osteomyelitis)? Usually, bacteria causes infection in the bone. Leukocytes are attracted to the area and secrete enzymes in an attempt to kill the bacteria. Blood flow to the area is decreased and a devitalized, necrotic bone is formed called sequestrum. A sequestrum is infected dead bone resulting from osteomyelitis. Haversian canals surround blood vessels and nerve cells throughout the bone. The sequestrum has no connection to normal bone through the Haversian system (osteon). Due to avascular nature of the sequestrum, antibiotics cannot reach the sequestrum or the bacteria. In fact, the bacteria enters the bone cells and hides inside the bone cells. The involucrum is new bone formation around the sequestrum. The body is trying to seal off the infection by forming new bone. The sequestrum will drain through the sinus. Biopsy of the sinus is not representative. Multiple deep samples are needed. Biopsy of the sinus is important in long standing cases of osteomyelitis to rule out squamous cell sarcoma.
Principles of surgical treatment for osteomyelitis:
Open the involucrum
Remove the sequestrum
Saucerize the bone: make sure a pathological fracture isn’t created. Stabilize if needed (external fixator is usually preferred)
Fill cavity with bone chips, cement or muscle flap if needed: antibiotics usually for 6 weeks (organism specific). Recurrence of infection is high – about 30%.

Chagas disease far deadlier than previously thought

A new study has found that Chagas disease may have been underreported as a cause of death.

The study, published in the journal PLOS Neglected Tropical Diseases, was led by researchers from the University of Sao Paulo in Brazil. The researchers sampled more than 8,000 people who donated blood from 1996 to 2000 and found 2,842 donors tested positive for Chagas disease at the time of blood donation. It was recorded that 159 of the Chagas-positive blood donors died during the 14-year time frame.

Those in the study with Chagas had a two to three times higher risk of dying, and 17 times more likely to develop cardiac disease, Ester Cerdeira Sabino, who co-led the study team, told CNN.

Chagas is transmitted by the triatomine bug, which is called the kissing bug because it bites sleeping humans near the eyes and lips. Kissing bugs defecate in or near the broken skin when they feed, transferring the Trypanosoma cruzi parasite into the bloodstream.

Chagas causes flu-like symptoms that subside after a few months, but it can reappear years or decades later as heart disease. The authors of the study found that Chagas was not reported as a cause of death in 42 percent of people in the study who died from cardiac disease, suggesting it has been underreported as a cause of death.

“What the parasite does to the body takes a long time; (it) slowly goes into the heart and destroys it,” Sabino told CNN. “We have measured accurately the risk of death, (as) a lot of mortality data doesn’t account for Chagas.”

Chagas has been identified by the U.S. Centers for Disease Control and Prevention as one of five neglected parasitic infections that require public health action. The CDC estimates there are 300,000 cases in the U.S. Kissing bugs have been reported in 28 U.S. states, mostly in the South.

There is currently no vaccine against the disease.

Mycetoma Clinical Trial Begins

The first major clinical trial for mycetoma – with a treatment developed by the Japanese pharmaceutical company Eisai – is raising hope for patients in desperate need of better treatments for this neglected disease. This is the first ever double-blind study for mycetoma, a disease so neglected that it only recently was added to WHO list of neglected tropical diseases.

The study is being conducted by DNDi and the Mycetoma Research Centre in Khartoum, Sudan, together with Eisai which is providing the trial drug fosravuconazole.

Those with gluten trouble should also be cautious with other grains because of proteins known as prolamins

Prolamins are proteins that exist in the seeds of cereal grains.


Gluten consists of a prolamin and a glutelin.

Prolamins are storage proteins, and they are the “hardware stores” of the wheat structure that distribute amino acids central to the plant’s development.

Glutelin is the “glue” that holds these hardware stores together.

Wheat gluten itself consists of the prolamin gliadin and the glutelin glutenin

Gliadin stimulates zonulin, the traffic cop of the gut lining, to allow it to pass into our digestive tract largely undigested and interacting with tissue transglutaminase (tTG), an enzyme found throughout the body

Tissue transglutaminase is a critical part of how the body functions, and specifically, how the tight junctions in the microvilli (think hairy fingers) in our gut are formed

When tTG interacts with gliadin, it creates antibodies – enzyme that is such an important part of our body is now bad – body attacks itself

Celiac disease is one of the most well-known gluten-related diseases, and those with the disease are advised to avoid more than wheat.
● Roughly 1% of people have celiac disease

Besides wheat, the following grains contain prolamins:

Grain Prolamin Protein
Wheat Gliadin
Oats Avenin
Corn Zein
Rye Secalin
Sorghum Karirin

Not all prolamins are believed to trigger celiac disease – the main ones are those from wheat, barley, and rye. Oats can be a trigger for some.

So how do prolamins and gluten relate?
● The prolamin gliadin in wheat is the main protein that is believed to trigger celiacs, and gliadin is one of the proteins that makes up gluten

Prolamins can cause more problems than just celiacs disease

For those who react negatively to them, they can cause:
● Brain fog
● Joint pain
● Thyroid issues
● Chronic inflammation
● Increase intestinal permeability

Many of the negative effects are due to a proinflammatory immune response to prolamins

In studies it was found that an increased production of proinflammatory cytokines occurred for both celiac patients as well as healthy controls, however the impact was greater for those with celiac disease.

Studies have confirmed that gluten-sensitivity exists, and many who experience IBS have gluten sensitivity

Case Study: In a double-blind, randomized, placebo-controlled study on 34 patients with IBS and without celiac disease, it was found that those who followed the gluten-free diet experienced fewer IBS symptoms, pain, fatigue, and bloating as compared with those who consumed the gluten diet.
● 34 patients
● 6 weeks of a gluten-free diet with exception of two bread slices and one muffin per day. Either with gluten or gluten-free
● The mechanism was not discovered as tests did not generate significant differences between the groups, however the reported symptoms were significantly less for those without gluten

If you have a sensitivity to gluten, it is a good idea to replace grains with starchy vegetables, such as sweet potatoes and beets

For everyone, it is a good idea to cut grains out for a week and reintroduce them one at a time to see if you have any sensitivity
